This policy (together with our terms of use and any other documents referred to on it) sets out the basis on which any personal data we collect from you, or that you provide to us, will be processed by us. YouTube cookies

We embed videos from our official YouTube channel using YouTube's privacy-enhanced mode. This mode may set cookies on your computer once you click on the YouTube video player, but YouTube will not store personally-identifiable cookie information for playbacks of embedded videos using the privacy-enhanced mode. We may collect information about your computer, including where available your IP address, operating system and browser type, for system administration and to report aggregate information to third parties. This is statistical data about our users' browsing actions and patterns, and does not identify any individual.

Storage of your personal data

By submitting your personal data, you agree to our storing and processing it. We will take all steps reasonably necessary to ensure that your data is treated securely and in accordance with this privacy policy. All patient identifiable information we collect is automatically encrypted using an industry standard AES algorithm and is stored on or within servers which are secure and comply with ISO 27001. Safety features to protect information against unauthorised access in the event a portable device with access to the Products is lost or stolen are in place and systems are capable of withstanding a determined and intelligent attempt to access the information without permission.

Where we have given you (or where you have chosen) a password which enables you to access certain parts of our site, you are responsible for keeping this password confidential. We ask you not to share a password with anyone.

The transmission of information via the internet is not completely secure. Although we will do our best to protect your personal data, we cannot guarantee the security of your data if being transmitted from your own device, or that of a third party transmitted to our site; any transmission is at your own risk. Once we have received your information, we will use strict procedures and security features to try to prevent unauthorised access.

By submitting data you agree to us using your information in the following ways:

If you are a registered user and have agreed to our patient terms and conditions, we will only contact you by electronic means (email or text) with information about services similar to those which were the subject of a previous agreement with you.

If you are not a registered user we will contact you by electronic means only if you have consented to this.

If you do not want us to contact you in this manner, please use the unsubscribe link provided on the bottom of all electronic communications from us.

Unless legally obliged to do so we do not disclose information about identifiable individuals to any other third parties, including but not limited to hospitals and pharmaceutical manufacturers, but we may provide them with aggregate information about our users (for example, we may inform them of the number of patients who had a particular type of operation at a particular hospital), only in such cases as there are no other contractual restrictions on our doing so.

Disclosure of your information to health care providers

We will only disclose information about registered users to third parties who are directly involved in delivering care, have a statutory right to access information with the correct authorities, or authorised by them, provided that registered users have consented to their information being used in this way.

The information collected may be used to produce data sets for the people and organisations responsible for the commissioning and provision of your health care services. The information may also be used to help further the research into both the condition for which you are receiving treatment and the design of the survey you have completed.

With your permission, the personal and survey data that you provide and other information will be held securely by PRO-MAPP who may provide the information collected to the parties involved directly in the commissioning and provision of your health care services.

By completing a PRO-MAPP questionnaire you are giving your consent for the information provided to be used for the purposes set out above.

As part of the survey process you will be asked to register with PRO-MAPP.  This is not compulsory but it will mean you agree that:

By completing a PRO-MAPP questionnaire, you are agreeing that your personal information may be shared with healthcare professionals involved in your care and on an anonymous basis for research. If you do not wish for your information to be shared with healthcare professionals involved in your care then please do not complete the requested questionnaire.

Your participation is voluntary, as is your registration with PRO-MAPP.  If you do not want to take part or register, do not fill in the questionnaire or register.  You may complete a questionnaire without registering but you may have to enter personal details again should you complete another PRO-MAPP questionnaire.

The quality of our products and services and the value of our service to our clients are paramount to us and PRO-MAPP will always strive to meet our clients' contractual requirements. We shall ensure a compliance review is carried out against our own processing activities as a Data Processor when supplying software products service solutions to our clients.

Your rights

You have the right to ask us not to process your personal data for any of the above purposes at any time by contacting us at  support@PRO-MAPP.com .

Our site may, from time to time, contain links to and from the websites of our partner networks and affiliates. If you follow a link to any of these websites, please note that these websites have their own privacy policies and that we do not accept any responsibility or liability for these policies. Please check these policies before you submit any personal data to these websites.

In the event of you instructing us to delete all data relating to you we will do so upon receipt of a Section 10 notice.  This is a communication from you to us explicitly requesting we delete all your information from our system.  We will also provide you with a list of Data Controllers in common (for example a provider involved in your care delivery) who you may need to issue a Section 10 notice to as well should you so wish, which will be subject to the third party's policy.

Access to information

The Act gives you the right to access information held about you. Your right of access can be exercised in accordance with the Act. Any access request may be subject to a fee of £10 to meet our costs in providing you with details of the information we hold about you.

Changes to our privacy policy

Any changes we may make to our privacy policy in the future will be posted on this page and, where appropriate, notified to you by email.
